I need a printable super bowl questionnaire. One that ask questions regarding who will win coin toss, etc?

I am looking for a questionnaire that I can print out for my party guest to fill out before the super bowl this sunday. I printed one out last year from the internet and this year can't find it. The questions ask guest which team or player is going to do/get things done first during the game. Does anyone know where I can find one to print out. thank you
